• The registration deadline for the Annual Meeting is September 6th, but we will continue to take registrations as long as space is available.
• Registration includes the Conference Program, Opening Reception, Past President's Banquet and all planned hotel meal functions.
• One completed form is needed for each attendee and/or guest.
• Please note that MEDA Membership is individual and not organizational.
• NOTE: If you want a printed copy of your completed registration for your personal records, click "Printer Friendly Page" before filling out the form, fill out the form from the Printer Friendly view, and then print before submitting.

Conference Cancellation Policy: Individuals who need to cancel for the conference must do so in writing no later than September 7th to receive a full refund. Attendees cancelling after this date will be subject to a $75 fee. No refunds will be given after September 13th or to attendees once the conference has begun. You may cancel via email to cjorae@medaweb.org.
